This is aimed at scanner output, where feeding a double-sided stack through leaves a blank page for every sheet that only had content on one side. Because a scanned blank is rarely pure white, the detection has an adjustable threshold so that dust, speckle and faint show-through from the other side are still treated as blank.

Those pages carry more marks than the threshold allows - usually scanner noise, a punch hole shadow or ink showing through from the reverse. Making the detection less strict will catch them.

A page holding only a faint mark, a thin line or a small logo can fall under the threshold. Tighten the setting so less is treated as blank, then re-run.

Does it work on a text PDF rather than a scan?

Yes, and it is more reliable there, because a genuinely empty page in a generated PDF has no content at all and no noise to work around.
